Telemedicine for Small Practices
During the Covid-19 crisis, telehealth is crucial for engaging your patients for new or follow-up visits. Medicare has relaxed its guidelines to enable this delivery of services.
What is Telehealth?
Telehealth visits are services provided via an interactive audio and video telecommunications system that allows for real-time communication between the provider and the beneficiary (except in AK and HI, where information can be transmitted and reviewed later and still be considered telehealth).

Medicare Telehealth

Medicare permits billing for some telephonic, virtual, and eVisit services and has expanded virtual visits to include new patient visits (see Medicare Billing Codes, below). Who may render/ bill for Medicare telehealth?
- Physicians (MD, DO)
- Nurse practitioners (NP)
- Physician assistants (PA)
- Nurse-midwives (CNM)
- Clinical nurse specialists (CNS)
- Certified registered nurse anesthetists (CRNA)
- Registered dietitians or nutrition professional (RD, DSME)
- Clinical psychologists (CPs)*
- Clinical social workers (CSWs)*
* CPs and CSWs cannot bill Medicare for psychiatric diagnostic interview examinations with medical services or medical evaluation and management services. They cannot bill or be paid for Current Procedural Terminology (CPT) codes 90792, 90833, 90836, and 90838. See the guide in “Billing Codes” below for more information.

Documentation
Documentation for a telehealth service should include:
- consent to telehealth services placed in the medical record;
- orders, updates to the care plan, and charges;
- location of patient and clinician during the visit;
- wording to note technology used and limitations;
- wording about follow-up; and
- the names and roles of any other persons participating in the telehealth service.
