Successful participation in the Quality Payment Program (QPP) requires practices and providers to undergo practice transformation.  To go through this process, practices need to first identify their “needs based on their local environment, opportunities…[and] their organizational capacity to evolve.”  Practices will also need to identify their outliers and gaps when it comes to high-risk patient care, and decide how to approach them in order to be more successful.
